What is endometriosis?

Endometriosis consists in the presence of endometrium, the lining that usually covers the uterine cavity, outside the uterus and it can affect the female body from the first menstruation (menarche) to the menopause.
Studies have highlighted that, in this specific condition, the endometrium has hormone receptors and a high adhesivity that enables it to adhere to extrauterine structures, where endometriosis develops.

Who is affected and what are the causes?

It is estimated that in Italy 10-15% of women of reproductive age suffer from endometriosis; the disease affects about 30-50% of women who are infertile or have difficulty conceiving. There are at least 3 million women with a clear diagnosis in Italy and around 190 million worldwide (WHO).
The majority is around 25 or 35 years old, but the disease may also appear in other age groups. The diagnosis often comes after a long and costly journey, usually experienced with serious psychological repercussions.

Different factors can cause endometriosis. It is currently believed that endometriosis appears due to:

  • Retrograde menstruation: when menstrual blood containing endometrial cells flows back through the fallopian tubes and into the pelvic cavity at the time that blood is flowing out of the body through the cervix and vagina during periods. Retrograde menstruation can result in endometrial-like cells being deposited outside the uterus where they can implant and grow.
  • Cellular metaplasia: when cells change from one form to another. Cells outside the uterus change into endometrial-like cells and start to grow.
  • Stem cells: they can give rise to the disease, which then spreads through the body via blood and lymphatic vessels. (WHO)

Other factors may also contribute to the growth or persistence of ectopic endometrial tissue. For example, endometriosis is known to be dependent on estrogen, which increases the inflammation, growth, and pain associated with the disease. However, the relationship between estrogen and endometriosis is complex since the absence of estrogen does not always mean the absence of endometriosis.

Treatments for endometriosis

Treatments to manage endometriosis can vary based on the severity of symptoms and whether pregnancy is desired. Even if there is no cure for the disease, a series of medications can help to manage the symptoms.
Physiotherapy can also be of great help, with manual therapy exercises and machines dedicated to the pelvic floor and pain management.

Endometriosis and radiofrequency

From 2019 to 2021, a study was conducted on 81 patients suffering from various diseases including endometriosis, chronic pelvic pain syndrome, adenomyosis, pudendal neuropathy, and others. The study showed that radiofrequency, by raising the temperature of deep pelvic structures using low-frequency electromagnetic currents, causes vasodilation and increased cellular activity. This promotes the repair process of connective tissue, improves its elasticity, and increases the pain threshold as it reduces inflammation. Last but not least, the study demonstrated a significant increase in the participants’ quality of life by 5 points
